If you are planning to volunteer for any of our school activities during the upcoming school year, please remember that you must have fingerprint clearance through the Archdiocese of Miami.

The Background Check Department
for the Archdiocese of Miami
will be at our school on Tuesday August 31, 2010.

Please know that you must register online prior to the appointment
date at www.adom.sofn.net.
The registration online must be done before August 31, 2010.

There is no cost to the parents for the fingerprints.

Dear Parents,

In preparation for the 2010-2011 athletic school year, Miami Children’s Hospital will be on campus to provide sports physicals for our student/ athletes. This opportunity will facilitate for our parents the option of saving time and money.

The cost of the sports physical is $30 payable to MCHF. If you should find this option feasible and convenient, please fill out the necessary forms on the school website.

The mobile unit will be on school grounds Tuesday August 31st, Wednesday September 1st and Thursday September 2nd.

This year, the deadline for ALL medical forms is September 10th. No child can participate in any after school sports without the medical forms.

If you'd like your child to be seen by the doctor, you must turn in the signed consent form, complete the frist page of the physical form ( Brief Medical History) and $30.00.
